What is ICAM ?
The Incident Cause Analysis Method (ICAM) is a structured approach designed to uncover all contributing factors to an accident or incident. By integrating human factors analysis, ICAM helps identify systemic organizational issues that may play a role in such occurrences.
Originally developed in the late 1990s, ICAM has since become a widely recognized and adopted methodology across various industries.

ICAM Process
The ICAM process is typically divided into several key steps:
Immediate Response
This stage focuses on implementing urgent measures to manage and control the incident, ensuring the protection of people and the environment.
Evidence Collection
Investigators gather a range of evidence, including physical items, documents, photographs, and witness accounts, to build a complete picture of the incident.
Data Analysis
The collected evidence is analyzed using ICAM’s structured methodology, categorizing contributing factors into four key areas:

Absent or Failed Defences – Controls that failed to prevent or mitigate the incident.

or Team Actions – Specific actions that directly caused or contributed to the event.

Task/Environmental Conditions – The circumstances, including human factors, under which the actions occurred.

Organisational Factors – Broader systemic issues within the organization that allowed these conditions to develop or persist.
Corrective Actions
Recommendations are made to address identified risks and prevent recurrence, which may involve updates to procedures, enhanced training, new equipment, or shifts in organizational practices and culture.
Reporting
The findings, conclusions, and recommendations are compiled into a detailed report, providing a clear and actionable summary of the investigation.
